It supports the use of compression as the ‘‘gold standard’’ for uncomplicated venous leg ulcers and the finding that multicomponent systems containing elastic bandages are more effective than those composed mainly of inelastic constituents. Importantly, it shows improvement in recent years in the methodological quality of included RCTs, which adds strength to the conclusions of the review. Consequently, research for management of venous ulcers must now surely shift to implementation strategies. This should focus on education and training of professionals, and improvement of healthcare resources that can support implementation. With an estimated 490 000 to 1.3 million European Union citizens having leg ulceration (all types), there are examples of best practice that could guide implementation and further improve patient outcomes. 2 This review identifies the need for future research to incorporate cost-effectiveness and quality of life measures within protocols. In the current constrained economic environment, such requests should be supported as this may influence not only current service delivery but future resource allocation for management of venous leg ulcers. The authors have provided a valuable resource of current evidence that will be of value to clinicians, policy makers, governing bodies, and patients alike. Multicomponent compression stockings in chronic venous leg ulcer treatment A review of the current literature.

The role of compression stockings in the treatment of chronic venous disease was amply demonstrated. Concerning patients with chronic venous leg ulcers (VLUs), multi-layer bandage is the standard of care. Recently, multicomponent compression stocking (McCS) systems for VLUs treatment and healing nowaday have been proposed. The aim of this review is to describe and analyze current literature evi...

Short-stretch or four-layer compression bandages: an overview of the literature.

Venous ulcers are a common, costly occurrence. Treatment typically includes the use of short-stretch and four-layer bandages--both with demonstrated ability to increase venous return and healing rates. Which is superior is unclear. To determine whether short-stretch bandages or four-layer compression systems provide shorter healing times and fewer adverse events when used in adults with venous ...

Clinical Inquiry: What is the best initial treatment for venous stasis ulcers?

the mainstay of initial treatment of venous stasis ulcers is compression therapy (strength of recommendation [SOR]: A, systematic review of randomized controlled trials [RCTs]). Multicomponent compression therapy is slightly superior to single-component therapy (SOR: B, systematic review of RCTs with inconsistent results). The various types of dressings available for managing venous stasis ulce...

Pentoxifylline for treatment of venous leg ulcers: a systematic review.

INTRODUCTION Venous ulcers are usually treated with compression therapy, but, because this treatment may not be effective for some people, adjuvant therapy could be beneficial. We did a systematic review of randomised controlled trials that compared pentoxifylline (with and without compression treatment) with placebo, or other treatments, in patients with venous leg ulcers. METHODS We identif...

Juxta CURES: An innovative method of providing compression for leg ulcer management

In the UK, it has been estimated that leg ulcers affect between 1% and 2% of the population (Graham et al, 2003). This equates to between 70 000 and 190 000 individuals with an open ulcer at any time (Posnett and Franks 2007). The prevalence of leg ulceration increases with age and is higher among women. Often, these individuals suffer with pain, social isolation and restricted mobility not to ...
